22.5 hours per week to be worked over 3 days MondayFriday between 9am and 6:30pm with flexibility for the successful post holder.£46,000 - £54,000 perannum pro rata depending on experience

Job summary

We arelooking to recruit an experienced, dynamic primary care pharmacist, independentprescribing qualification is desirable but not essential although should bewilling to work towards this. The role will be based in our practice alongsideour clinicians including pharmacists already in surgery deployed from our PCN.

We arelooking for a practice pharmacist to work closely with the team to ensure wedeliver an effective, efficient and quality medicines management service to ourpatients and our team.


Main duties of the job

ThePharmacist will undertake the following tasks.

Medicationreviews Face to face, remote and e-consultations) and proactively managepatients (some with complex polypharmacy). This will include patient education,prescribing and monitoring as per guidelines and within professionalcompetencies.

Workclosely with the wider team, advising and supporting on medicine managementissues.

Clinicalinput regarding the supporting GPs with medication titration.

Undertakeprescribing audits.

Ensurethat the practice is encouraged to promote cost effective prescribing.

Ensuremedicines are prescribed and monitored as per the most current guidelines.

Ensurethat the practice systems for drug safety and drug safety monitoring are adheredto at all times. This work will include regular safety audits.

Shareknowledge and learning with the wider teams.

Attendregular practice meetings.

Representthe practice at outside events.

Contributeto achieving high IIF/ QOF targets.

Undertakemedication reviews for housebound/care home patients as needed.

Take anactive role to help/ support practice initiatives.

Carryout structured medication reviews on patients with complex polypharmacy,implementing prescribing decisions and requesting appropriate tests.

Reconcilemedicines following hospital discharge and work with patients and the communitypharmacists to ensure that the patients receive their medication postdischarge.

Reconcilemedicines from clinic letters with the support from the wider practice team.

Supportlong term condition reviews via face to face and telephone appointments.

Contributeto reductions in medicine related hospital admissions and re-admissions bysupporting patients to get the best outcomes from their medicines.

Identifyand address medicine related issues.

Assistin managing patients and healthcare professional medicine queries.

Increasepatient quality and patient safety of prescribing through mechanisms such asaudit and PDSA cycles. The share the findings with the wider team.

Contributeto service improvement within enhanced services and QOF

Implementdrug withdrawals and alerts eg MHRA

Workwith the primary care professional and patients to implement NICE and otherevidence based guidelines.

Act as asource of medicine information for all of the practice team and patients egaround supplies, doses, side effects


Person Specification


Experience

* Able to demonstrate practical experience of dealing with patients.
* Evidence of Continuing Professional Development
* Experience of working in General Practice
* Has an awareness of a breadth of common acute and long-term conditions likely to be seen in primary care.


Qualifications

* Member of the Royal Pharmaceutical Society (RPS)
* A UK accredited MPharm degree or OSPAP qualification.
* Clinical Pharmacy Diploma
* 52 weeks of pre-registration training in England, Scotland or Wales signed off as satisfactory by your tutor.
* The GPhC registration assessment with a pass result.
* To be responsible for own professional development and maintain competence withing own clinical field.
* Hold an independent prescribing qualification
