* To support the development of enhanced clinical skills in end‑of‑life care, ensuring access to appropriate development to maintain a competent workforce.
* The post holder will provide mentorship and work‑based learning facilitation, in‑house curriculum design and teaching in end‑of‑life care.
* To perform an expert clinical advisor and leadership role and professional support for the community teams, to ensure that safe, effective, high‑quality evidence‑based care is delivered in an efficient and cost‑effective manner, ensuring that performance management targets are met and evidenced through data production and analysis.
* Developing clinical governance through the support, monitoring and implementation of enhanced clinical standards and learning within the clinical services.


Responsibilities

* Act as a role model for clinical staff to address patient needs and ensure the delivery of safe patient care.
* Keep accurate, contemporaneous records and written reports as required.
* Report and proactively support training and development issues arising from clinical and non‑clinical incidents.
* Proactively manage risk to vulnerable persons and support training and development of staff identified through development plans.
* Participate/facilitate meetings in response to the clinical development needs of staff.
* Work collaboratively with clinical professional colleagues and agencies to ensure effective communication and identify professional practice development needs.
* Manage risk and maintain service delivery through effective communication and liaison with Service Managers, staff and professional colleagues.
* Engage constructively with clinicians and students to support, develop and enhance quality standards of clinical practice in the work environment.
* Provide clinical leadership, development and advice to clinical staff, patients and carers based on current evidence‑based knowledge on end‑of‑life care.
* Participate in clinical supervision of registered professionals as required by the clinical supervision policy.
* Maintain up‑to‑date clinical skills and knowledge in end‑of‑life care and stay aware of professional issues through research and evidence.
* Demonstrate personal accountability and an understanding of responsibility for staff to which actions are delegated.
* Contribute to the improvement of patient care and patient safety by effectively disseminating and integrating evidence‑based guidance into clinical practice, across physical health, mental health and learning disabilities.
* Incorporate patient preferences into treatment decisions, including advanced directives near the end of life and recording on clinical systems (RiO, LCR).
* Embed the Oxleas NHS Foundation Trust Core Values.
* Act as a clinical and educational expert in end‑of‑life care, leading in clinical training, education and programmes.
* Identify learning needs, develop education programmes and manage competing demands of practice and education.
* Evaluate the effectiveness of education and its impact on service delivery and patient care through audit and benchmarking.
* Facilitate learning by integrating theory into practice and create a positive learning environment supporting lifelong learning.
* Provide support for learners, mentors and specialist practice students (community nursing pathway).
* Promote and facilitate understanding of relevant national and local policies in end‑of‑life care.
* Develop an end‑of‑life skills database and career/professional development pathways.
* Work closely with the Head of Nursing to support professional recommendations for service redesign and new ways of working.
* Act as a change agent, negotiating and training to facilitate the change process.
* Ensure effective and efficient use of resources, equipment and supplies.
* Practice in accordance with competency level and maintain professional registration.
* Demonstrate ongoing professional development and participate in the Trust appraisal process.
* Facilitate the collection of data.


Qualifications & Experience

* Registered Adult Nurse with community nursing experience at Band 6 or equivalent.
* Minimum two years working at Band 6 or equivalent in an acute or community setting.
* Experience in end‑of‑life care or equivalent qualifications.
* Experience of working collaboratively and negotiating across organisational boundaries.
* Experience of change management and implementation in practice.
* Teaching/mentoring experience and experience of receiving and delivering clinical supervision.
* Ability to undertake an audit.
* Evidence of continuing professional development: Nursing Degree and SPDN Qualification (essential), MSC/Degree (desirable).
* Teaching and assessing qualification (essential).
* Enhanced clinical assessment skills (desirable).
* Palliative and EOL care training or courses (desirable).


Payroll & Contract Information

* Pay scheme: Agenda for change
* Band: Band 7
* Salary: £53,751 to £60,651 a year pa inc
* Contract: Fixed term (12 months)
* Working pattern: Part‑time, Job share
