Practice Nurse – West Cornforth Medical Centre
Provide basic practice nursing services to the practice population. Supported by senior nurses, deliver care within the boundaries of the role, focusing on supporting patients to be healthy, monitoring long‑term conditions, health prevention and screening activities. Work collaboratively with the general practice team to meet patient needs, support delivery of policy and procedures, and provide nurse leadership as required.
Key Responsibilities
* Promote health and well‑being and prevent adverse health effects.
* Implement and evaluate individual treatment plans for patients with known long‑term conditions.
* Identify and manage treatment plans for patients at risk of developing long‑term conditions.
* Prioritise health problems and intervene appropriately in complex, urgent or emergency situations, including initiation of effective emergency care.
* Support patients to adhere to prescribed treatments and adopt self‑care strategies.
* Deliver opportunistic health promotion during consultations (e.g., new‑patient medicals).
* Assess and care for uncomplicated wounds.
* Support and manage health needs of women presenting for cervical cytology consultations.
* Implement and participate in vaccination and immunisation programmes for adults and children.
* Advise, support, and where appropriate, administer vaccinations for patients travelling abroad.
* Utilise sensitive communication styles, ensure patients are fully informed, and manage communication barriers.
* Record clinical notes accurately and maintain health records in accordance with legislation and practice policy.
* Maintain professional competence in line with Nursing and Midwifery Council standards.
Qualifications and Experience
* Registered General Nurse with current NMC registration and revalidation.
* Minimum two years of practice nursing experience (or at least one year in general practice).
* Experience with chronic disease management, diabetes care, and first contact/telephone triage.
* Experience managing long‑term conditions and delivering vaccination programmes.
* Knowledge of NICE, national and local guidelines, and QOF/KPI quality initiatives.
* Competence in venepuncture, ECG, wound care, adult/children vaccination, cervical cytology, ear irrigation, spirometry, NHS health checks, and cryotherapy.
* Experience of treatment room duties and patient group directions.

* Competence in information management, data retrieval, and use of clinical systems.
Professional Development & Additional Requirements
* Maintain continuous professional development and undertake mandatory and statutory training.
* Work independently without direct supervision where required.
* Subject to a Disclosure and Barring Service check under the Rehabilitation of Offenders Act.
