Responsibilities
* Be professionally & legally responsible for all aspects of your own work and for the management of patients in your care, delivering a high standard of clinical care and supporting junior staff.
* Deliver a SLT service to stroke in-patients referred with swallowing and/or communication problems; assess, develop and implement Highly Specialist SLT treatment for stroke inpatients.
* Provide expert advice to the MDT and patients/families in complex or sensitive cases; participate in Nutrition MDT, attend ward rounds, board rounds, family meetings and team meetings as required.
* Write reports reflecting highly specialist knowledge; support smooth transition of care across the stroke pathway; liaise with community and Early Supported Discharge teams; contribute to discharge planning and evaluate outcomes.
* Demonstrate clinical effectiveness using evidence-based practice and outcome measures; involve patients in planning and prioritisation of their care; negotiate with professionals on case management in complex cases.
* Adapt practice to individual patient circumstances, including cultural and linguistic differences, and consider emotional and psychological factors affecting intervention.
* Contribute to service development and policy, including development of stroke team objectives, protocols, and national guideline adherence (e.g. NICE, RCP Stroke Guidelines, SSNAP metrics); monitor and review service delivery and reflect best practice locally and nationally.
* Lead and supervise a team of SLTs, Apprentices and assistants; provide regular specialist training on dysphagia, communication and cognitive linguistic disorders; support supervision and problem solving within peer supervision.
* Provide student placements, assess placements, and coordinate observation and training for students from other professional groups; participate in CPD development and staff recruitment/induction as appropriate.
* Coordinate ASU SLT rotation (including Sunday rota) to provide a consistent service; ensure up-to-date and accurate case notes in line with professional standards and local policies; share information with data protection compliance.
* Engage in risk management, quality standards development and clinical governance; participate in departmental research, audits and data collection (e.g., SSNAP data) and contribute to interagency multidisciplinary teams.

* Communication and relationship skills: discuss input with clinical teams; communicate complex information to clients, carers and families; demonstrate empathy and effective communication, especially where barriers exist; motivate and negotiate with clients and carers.
* Ability to form productive relationships under stress, manage conflicts, and uphold professional boundaries; demonstrate evidence-based practice and use of outcome measures.
