Role Overview: As a Newley Qualified Social Worker in the Adults with Learning Disability, Autism Team and Transition Team you will play a crucial role in supporting adults 18 through adulthood. Your responsibilities will include: Case Work: Conducting Mental Capacity (MCA) Assessments, Court of Protection applications, and s42 safeguarding enquiries. Positive Risk Support: Encouraging individuals to take positive risks while aiming for the least restrictive interventions. Collaborative Working: Partnering with service providers and stakeholders to deliver comprehensive support. Strength-Based Approach: Undertaking assessments, setting personal goals and outcomes, and reviewing their effectiveness. Person-Centred Decisions: Ensuring the individual's wishes are at the heart of every decision. Resource Management: Exploring community and commissioned services to meet eligible needs within budget.
