Team Leader (Supported Living)
* Monday to Friday, 8am to 4pm.
* Occasional evening and weekend cover depending on the needs of the business.
* Not directly providing support, unless covering a shift.
* Pay Rate £15.00 per hour
The Team Leader will provide practical and emotional support to individuals with autism, learning disabilities, health problems, mental health needs, and emotional difficulties and to their support team. They look after the well-being of vulnerable people by ensuring that the team are available, skilled and equipped to assist people with day-to-day tasks and enabling them in ordinary living skills at home, in the community, in recreation and leisure.
Team Leader Job Description:
You will be caring and empathetic, assisting people living in their own home with day-to-day tasks. Work with individuals to assess their practical and emotional needs, contribute to support and activity plans, and help them to live as independently as possible. You may also be required to contribute to multi-disciplinary meetings with other professionals.
You will possess excellent interpersonal skills, a passion for helping others, and good leadership skills. Providing both emotional and physical support when needed to ensure that people feel in control of their lives.
Team Leader Responsibilities:
* Manage a staff team including rotas, time sheets, supervisions, appraisals, and team meetings.
* Participate in an on-call rota to ensure that support is provided in accordance with contracts.
* Induction and training of staff.
* Ensure staff are caring, supporting, respectful and personalised support is always provided.
* Ensure Code of Practice is adhered to and support is person centred and individually led, promoting choice and active participation.
* Hold regular staff meetings, complete staff supervisions on a regular basis.
* Provide outcome focused and personalised support plans for the people we support, that best suit the individuals' needs, including risk management and enablement.
* Promote a culture of active support, engagement in the community and promotion of independent fulfilled lives.
* Carry out support in line with PATHs.
* Ordering, receiving and supporting people with managing their medication.
* Arrange and attend Tenants house meetings.
* Hold review meetings with the people we support, advocates, families, and commissioners.
* Raise safeguarding issues when required.
* Resolve complaints.
* Work with housing providers to ensure accommodation is maintained to a high standard.
* Ensure maintenance is carried out and any health and safety checks are performed in line with requirements.
* Ensure CQC regulations are met, including sending notifications when required.
* Meet and work directly with people we support to agree and provide support with their practical and emotional needs.
* Assist with domestic tasks including shopping, cooking, cleaning, and washing, as well as providing personal care.
* Ensure the healthcare needs of each person are met appropriately.
* Help people to complete their paperwork and manage their individual finances.
* Support people with drug or alcohol addictions.
* Ensure that medical and other appointments are made and support people to attend.
* Be responsible for the well-being and safety of people supported.
Team Leader Requirements:
* Relevant experience in Health and Social Care and especially Mental Health, Autism, ADHD.
* Relevant experience in supporting people with complex needs and behaviours.
* A management qualification in health and social care is desirable.
* Commitment to helping others.
* Excellent communication and interpersonal skills.
* Empathetic with good listening skills.
* Good problem-solving abilities.
* Ability to carry out domestic duties alongside the people you support and develop additional independent living skills.
* Willing to assist with basic health care practices.
* Patient and emotionally resilient.
* Strong leadership skills.
* Able to work consistently, reliably and as part of a close team.
