About the Role
We are seeking a detail-oriented and organized Care / Case Manager to support patient onboarding, test coordination, and client management operations within a growing integrative medicine practice.
This role is responsible for ensuring smooth intake processes, accurate test ordering, and organized tracking of patient information. The position requires strong written communication skills, attention to detail, and the ability to work independently in an asynchronous remote environment.
The ideal candidate is dependable, efficient, and committed to maintaining high standards of organization and customer service.
Core Responsibilities
Patient Onboarding
Guide new patients through intake forms and initial setup
Ensure all required documents are completed and organized, using an EHR system.
Provide clear, timely communication regarding next steps
Test Ordering & Coordination
Assist with ordering functional and specialty lab tests
Track lab orders and results
Communicate updates to patients professionally and clearly
Client Management & Organization
Maintain and update client management spreadsheets
Track patient progress within internal systems
Ensure accuracy and attention to detail in all records
Communication & Customer Service
Respond to patient emails/messages in a professional and warm tone. Provide logistical support (not medical advice)
Maintain timely communication and clear boundaries
