We are seeking an experienced and dedicated Senior Social Worker to join our dynamic team – Caradon CMHT within the Cornwall Partnership NHS Foundation Trust Mental Health Service. This community-based role will deliver high‑quality, person‑centred care and involve frequent travel within the county.

Main Duties of the Job
* Assess care needs and provide clinical management for a defined caseload within the CFT Mental Health Service.
* Carry out continuing responsibilities for the development, implementation, and evaluation of programmes of care.
* Develop the interface between primary and secondary care services to provide focused and coordinated care packages tailored to patients and carers.
* Travel frequently to various locations across the county.

Person Specification
* Relevant professional registration – Social Worker.

The post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and requires submission for Disclosure to the Disclosure and Barring Service.
