Our caring and empathetic care workers are responsible for the health and wellbeing of our clients. The responsibilities of this position include assisting with activities of daily living, medication management, monitoring clients' health, and keeping a detailed logbook of clients' daily activities.
Responsibilities:
* Give emotional support to people with mental illness or cognitive disabilities.
* Monitor physical health status and make medical referrals.
* Coordinate services for patients with multiple health care providers.
* Ensure that patients follow the treatment plan or prescribed medications.
* Provide companionship for patients who have no one for social interaction.
* Manage schedule of activities for groups of patients.
* Provide assistance with daily living tasks such as bathing and dressing.
* Assist mentally ill or cognitively disabled patients to prepare meals, shop, or perform other daily tasks.
