General Practitioner (Additional Roles Reimbursement Scheme)
The post-holder will manage a caseload and deal with a wide range of health needs in aprimary care setting, ensuring the highest standards of care for all registered and temporarypatients.
Your contractual base will be Middlestown Medical Centre (which will also be used for thepurposes of claiming mileage) but you will be required to work flexibly across the PCN GPpractices and premises and potentially across the Wakefield District.As the role requires you to commute across the Wakefield District applicants should eitherhold a full driving licence, which is valid in the UK, and hold the relevant car insurance to covertravel for business purposes or have the ability to be mobile by using other modes of transportthat will allow you to get to the destination within a reasonable timescale. Reasonable expenses will be payable for journeys outside of your regular commute distance to your basepractice.
Hours of work are up to 6 sessions, 27.75 hours (0.74WTE) to be worked flexibly between 8amand 6.30pm Monday to Friday. Where you may be required to support clinical services outsideof these hours, time off in lieu will be made available

We are a dynamic, forward thinking PCN team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients and reducing health inequalities.
We are looking to expand our team by recruiting a team of General Practitioners to work alongside our practices and PCN staff. Our PCN staff team currently include clinical pharmacists, pharmacy technicians, social prescribers, health and wellbeing coaches, care co-ordinators, nurse associates, occupational therapists, mental health nurses, and general practice assistants.
Job responsibilities
In accordance with the PCN/General Practice timetable, as agreed, the post-holder will make themself available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self- referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.
Providing counselling and health education.
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
Recording clear and contemporaneous consultation notes to agreed standards.
Collecting data for audit purposes and complying with QOF requirements.
Delivering enhanced services.
Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).
Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Awareness of and compliance with all relevant PCN and Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety.
A commitment to life-long learning and audit to ensure evidence-based best practice.

Contributing to the summarising of patient records and Read-Coding patient data.
Attending training and events organised by the Practice or other agencies, where appropriate.
