This is an exciting time to join our District Nursing service, as we continue to develop in response to the changing needs of communities, and in line with the NHS Long-Term Plan. We are currently working with our partners towards improving both the delivery and access to our services including the development of Integrated Neighbourhood Teams. The successful person will be a qualified District Nurse with the Independent Non-Medical Prescribing qualification (V300) or willing to undertake. By having these advanced clinical skills the ambition is to plan and deliver advanced care to patients in the place they call home, preventing unnecessary hospital admissions. Main duties of the job As the Clinical Team Leader, you will be responsible for the provision of excellent nursing care delivery to a diverse population of patients. You will be expected to case manage patients and to demonstrate your clinical credibility by supporting and supervising staff with their clinical workload, provide clinical expertise for complex patient care, whilst line managing your team. You will have excellent communication skills and candidates will be expected to demonstrate a proven track record of dealing with complex situations including the sensitive handling of complaints, being active in high level meetings as well as being able to present and articulate clinical information in relation to the Stop the Line Process and Datix investigations. You will work alongside colleagues to improve standards of care for people in Wakefield and you will be supported and encouraged to develop your team to the highest clinical and professional standards. Qualifications Essential • Registered Nurse qualification. • Specialist Community Practitioner District Nurse Qualification. • Be able demonstrate completion of preceptorship period. • Independent Nurse Prescribing Qualification or willingness to undertake.
