Responsibilities
* Work closely with existing stroke and neuro services, social care provision and voluntary sector, with support from a multidisciplinary team and access to clinical supervision and peer support.
* Provide a specialist occupational therapy service to patients recently diagnosed with a stroke or neurological condition in a range of community settings, using specialist practice underpinned by theoretical knowledge.
* Serve as a specialist resource of information for health and social care staff, patients, families, carers and external agencies who may require advice, support and guidance in the management of stroke and neurological conditions.
* Transfer individual patients’ ongoing care to other agencies as appropriate and ensure that the service provided is conducted in a professional, efficient and effective way.
* Meet the service specification objectives and key performance indicators as part of a multi‑disciplinary team.
* Provide a comprehensive specialist assessment of patients referred to the service, often with complex presentations, in a holistic manner using investigative and analytical skills.
* Apply advanced clinical reasoning to devise effective rehabilitation treatment plans, seeking guidance from the senior occupational therapist where applicable.
Qualifications
The successful candidate will have a wide range of clinical experience across the stroke and neuro care pathway, an understanding of collaborative working across multiple agencies, and a passion for delivering pathways of care that meet high clinical standards and the needs of individual patients, families and carers.
