Reablement Assistant Practitioner - 7117 | Permanent contract | £31,537 to £33,699 per annum (Scale G) | 37 hours per week | Priory House, King’s Lynn
Would you like to work in the community as part of our well-trained team of Occupational Therapists and support workers, creating person centred personal plans to help people set and achieve their goals following an illness? This role involves visiting people in their own homes to assess social care needs and support their recovery and independence.
Responsibilities
* Visit people in their own homes to assess social care needs across a range of locations.
* Work as part of a multidisciplinary assessment team with Social Care professionals to support people discharged from hospital or with changes in circumstances requiring home support.
* Assess daily living needs and develop personalised plans to help individuals regain skills and confidence to remain independent at home.
* Complete a Care Act Assessment when ongoing services are required. All training will be provided to ensure you are competent to deliver this role.
* Communicate clearly and appropriately with service users and the public, exercising judgement and using initiative to plan and prioritise work.
* Demonstrate knowledge of current policy, practice and legislation within Health and Social Care.
Requirements
* Experience of working with people and effective communication with members of the public.
* Ability to work within a 7-day rota and to schedule work in advance to support work-life balance.
* Willingness to learn and apply relevant Health and Social Care policy and legislation.
