Job Overview

An exciting opportunity has arisen for an enthusiastic and motivated Community Nurse/Health Facilitator to join the Adult Community Learning Disabilities Team (CLDT)Trafford, based at Sale Waterside.

The CLDT is a well-established and friendly team comprising of nurses, speech and language therapists, occupational therapists, physiotherapists, psychologists and psychiatry.

Nursing is a valued and respected component of the team. The team also links closely with our colleagues in Greater Manchester, as part of a wider Greater Manchester Network.

The Community Nurse will work as part of the multi-disciplinary community support team, and will be expected to work autonomously, managing a defined caseload of adult service users with Learning Disabilities and complex health needs.

The post requires an enthusiasm and commitment to working with adults with learning disabilities within a person-centred approach.

Main duties of the job

Managing a caseload under supervision, the post holder will be expected to work autonomously and as part of the team to improve outcomes for adults with Learning Disabilities within their local community, whilst maintaining quality lives and ensuring people are free from harm.

Applicants should possess excellent interpersonal, communication and literacy skills and the ability to work with a range of professionals, agencies and carers in a wide range of settings. You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.

Development opportunities will be provided in the form of training and you will receive regular clinical & management supervision and annual appraisal.

Person Specification Qualifications Essential Criteria


* First level Learning Disability Nursing qualification
* Evidence of post-registration training
* Evidence of post-registration training in specialist area to diploma / degree level
* Demonstrates commitment to ongoing training and personal development

Desirable criteria

* SPC qualification

Knowledge and Expertise Essential criteria

* Direct experience of working with clients with learning disabilities and complex needs
* Experience of working collaboratively in a multi-disciplinary setting
* Experience / ability to work independently
* Awareness of the health access agenda, relevant legislation and government policy relating to learning disability
* Ability to assess, plan, co-ordinate and evaluate care
* Ability to chair meetings and lead a team i.e. CPA
* Ability to identify areas in nursing practice for development and support others to develop and change practice
* Ability to deliver training and make formal presentations to groups of professionals, service users and service providers

Desirable criteria

* Developed good practice, guidelines / protocols / policies
* Previous examples of leading to implement new practice
* Special interests related to area of practice

Experience Essential criteria

* Minimum 1 year post-registration experience working in multidisciplinary health teams
* Direct experience of working with vulnerable groups who have learning disabilities and additional health needs
* Experience of multi-agency working and ability to work effectively across service boundaries
* Direct experience of working with clients with complex needs i.e. challenging behaviour, mental and physical health problems.
* Delivery of training to service users, families and carers
* Awareness of relevant legislation and good practice relating to learning disabilities (Mental health Act, Mental Capacity Act, Transforming Care, National Mortality Review etc.)

Desirable criteria

* Experience of working on service developments
* Participation in service developments
* Experience in delivery of quality Initiatives
