Job overview
* To undertake medicines reconciliation and advise on medicines management within inpatient services.
* To work in partnership with patients and other professionals and staff within other organisations to maximise effectiveness and minimise the risk associated with medicines on admission and at discharge.
* To promote independence with medicines.
* To promote best practice in medicines management within inpatient services.
* To promote the seamless transfer of pharmaceutical care across the primary / secondary care interface by improving medicines management systems.
* To develop the role of the Administration Pharmacy Technician for the benefit of the patient.
* To provide support and training on medication management to ward staff, including student nurses, on behalf of the ward.
* To administer prescribed medication to patients as per competency.

Person specification
Qualifications
* BTEC in Pharmaceutical Sciences / NVQ in Pharmacy Services level 3 or equivalent plus additional training in checking dispensed prescriptions.
* Registered with the General Pharmaceutical Council (GPhC).
* Experience of managing medicines to conform with legal and procedural requirements.
Experience
* Working in a Mental Health setting
* Experience of rehabilitation
* Delivering group/ exercise sessions
* Multi-Disciplinary Working as a member of an integrated team
* Experience and knowledge of falls and falls prevention
* Exposure to older adults, including falls, frailty and mobility rehabilitation.
* Exposure to working-age adults with complex physical health needs (e.g. neurological, musculoskeletal).
Knowledge
* Appropriate demonstrable post-qualification pharmacy work experience, covering the main skills areas, and including the relevant speciality.
Skills
* At least part of this experience to have been within a Hospital / Community Pharmacy department.
* Able to use own judgement to prioritise tasks.
* Audit of practice and / or of the impact of a change in the way the service is delivered.
* Experience in objectives setting for a team.
* Good communication and interpersonal skills.
* Contributed to the implementation of changes in service, products and / or systems.
* Problem solving.
* Microsoft Office skills, including Excel and data analysis
* Supervision and mentoring of junior staff
