Main duties of the job

Working to support our Care Leavers in Hull with our NHS and local authority partners is arguable more crucial than ever. Job responsibilities

Socialprescribing empowers people to take control of their health and wellbeingthrough referral to non-medical social prescribing link workers who give timeto focus on what matters to me. This role specifically works with youngpeople, taking a holistic approach connecting them to community groups andstatutory services that provide emotional and practical support.

You will work with other agenciesand partners to support care experienced young people aged sixteen - twenty five, and those atrisk of being not in employment, education and training, to improve theirwellbeing, help them to live independently and improve their health outcomes byconnecting them into a broad range of services and interventions. Reasons forreferral may include, but not be limited to, mental health difficulties,financial support, isolation, family issues, friendship issues, support withnavigating the healthcare system, anxiety and low mood. The post holder shouldtherefore have experience of working within or alongside the care system, orwith young people who are care experienced. The post holder should have a goodunderstanding of mental health challenges experienced by young people, and havean understanding of trauma informed care. The work will be led by the wants andneeds of the young people themselves, and may need some flexible working hoursto fit the availability of the young people.

The Young PeoplesSocial Prescribing Link Worker will work closely with partners across Hull,including the Leaving Care Team, Connexions Team, Hull City Council, PublicHealth, and others.

Key Areasof Responsibility

To act as a main pointof contact for young people accessing the service.

Accept referrals and assess how far a patientshealth and wellbeing needs can be met by services and other opportunities inthe community.

Manage and prioritise your caseload reflecting yourpatients needs.

Together produce a simple personalised care andsupport plan to address their health and wellbeing needs.

Develop trustingrelationships by giving people time and focus on what matters to them.

Following a patients journey, evaluate how far theactions in the care and support plan are meeting their health and wellbeingneeds.

Where required refer patients back to referees withsignposting suggestions.

Liaisewith appropriate statutory and voluntary sector agencies working in Education(including SEND), Public & Mental Health, Primary and Secondary care,Social Care and Youth Offending Teams and build up appropriate networks thatpromote timely access to support for young people.

Towork systemically, e.g. liaise with and include parent/carers, educators,health professionals etc. when appropriate and as agreed by the service users.

Tobuild strong working relationships with internal and external services and topromote the link worker/social prescribing model.

Work collaboratively with local Voluntary, Communityand Social Enterprise (VCSE) organisations and community groups to receivesocial prescribing referrals.

Ensure data is captured in the clinical systems andSNOMED codes are utilised.

Discuss concerns with a relevant GP to ensuresafeguarding procedures are followed appropriately.

Toattend multi-disciplinary team meetings (MDTs) as required.

Toensure young people are made aware of key processes e.g. complaints,information sharing, getting involved, and to make sure that they are askedabout how they would prefer information.

For your own personal and professional development,you will be expected to participate in monthly supervision with a healthprofessional.

Toattend mandatory training and abide by policies and procedures on safeguarding,health and safety and equality and diversity.

Toattend appropriate continuing professional development and training events,team meeting and supervision sessions.

To carry out, from time to time, any otherappropriate duties in line with capabilities as directed by the line manager.

* Drive and have access to transport to attend visits or sites across the Hull, this may include home visits


Experience

* To deliver outcomes based on set targets
* Liaise with other colleagues to promote the success of the YHP and its partner organisations in primary care
* Work to perpetuate a culture that is transparent and open
* A good listener who demonstrates empathy
* Has the skills to assess risk and identify ways to mitigate these
* Awareness of and has well-developed IT skills to facilitate quick and confident learning of new systems to support the management of patients in a primary care setting (including Microsoft packages)
* Working well with others to deliver goals
* Being motivated to deliver excellent services to support Primary Care in our region
* Experience of working within a community, social care, or voluntary sector
* Has experience of supporting people in a paid or unpaid capacity
* Has previous experience of working in a Care Leavers setting.
* Has previously worked in a Primary Care setting
* Has managed a patient/client case load
* Has produced care plans for individuals
* Communicates with colleagues well to ensure the smooth delivery of services
* Proactively supports others to deliver team objectives
* Takes responsibility for actions and looks to enhance the service and its reputation in all activities undertaken

Qualifications

* Has undertaken the NHS England and NHS Improvement online learning programme
* Enrolled/in progress/qualified from appropriate training as set out by the Personalised Care Institute
* To understand and in practice apply Caldicott Principles, Data Protection Act 2018 and appropriately protect and individuals personal data, disclosing where appropriate only
* Demonstrable commitment to CPD
* Understands person-centred approaches
* Has knowledge of Trauma Informed Care
* Aware of voluntary sector, community groups and statutory services locally
* Has safeguarding knowledge either level 2 or the equivalent experience
* Training in motivational coaching and interviewing or equivalent experience
