Senior Clinical Fellow (SPR) in Heart Failure / Transplant Medicine

This is a registrar‑level post in Advanced Heart Failure/Transplant Medicine; the successful applicant will join a dynamic team with an international reputation. Harefield Hospital provides specialist cardiothoracic services including interventional cardiology, thoracic and cardiac surgery. The post commences in December for a period of 6 months with a possible extension by mutual agreement (up to a maximum of 23 months where appropriate).
This post offers clinical experience and training in advanced heart failure, ventricular assist device support, and clinical management after heart transplantation. It is suitable for those seeking a post‑CCT fellowship in advanced heart failure and for current trainees in cardiovascular medicine who wish to have a period of out‑of‑programme training in advanced heart failure.
The post holder will undertake an active role in inpatient care and participate in outpatient clinics as well as the EWTD‑compliant on‑call rota.

Main Duties
 * Work as a full‑time medical team member of the Trust's multi‑disciplinary transplant team.
 * Provide clinical management and support to heart failure patients receiving mechanical circulatory support, primarily as a bridge to transplantation.
 * Provide clinical teaching to medical team members, with particular emphasis on management of advanced heart failure, ventricular assist devices, and post‑transplant long‑term care.
 * Conduct clinical audit and retrospective analysis of clinical data, with emphasis on training and mentoring junior team members.

Clinical Responsibilities
 * Work as a full‑time clinical member of the multi‑disciplinary team, including participation and presentation at weekly MDT meetings and other meetings in the division and Trust.
 * Ensure good communication is maintained within the Transplant service at Harefield Hospital, working in collaboration with medical and surgical colleagues, other departments and services.
 * Assist when required in the Angiography Laboratory.
 * Attend and lead outpatient clinics.
 * Arrange for the transfer of patients to/from other hospitals or discharge home, including a transfer/discharge summary.
 * Perform minor surgical procedures such as chest aspiration, chest drain insertion, central venous lines and arterial blood gases etc.
 * Supervise and train junior staff allocated to the Transplant Service.
 * Undertake regular ward rounds to ensure effective management during the in‑patient stay.
 * Keep the Ward Sister and Consultant informed of any patient giving cause for concern.
 * Communicate to a patient's relative the progress of that patient.
 * Dictate/input all urgent discharge summaries only and ensure that these are forwarded to the patient's General Practitioner or hospital of transfer as soon as possible after discharge.
 * Be available for emergency calls to ITU and other parts of the hospital as required (e.g. cardiac arrest).
 * Participate in the Medical Audit Programme and update relevant patient database programmes.
 * Participate fully in the service and training programme of the Transplant Service and Heart Division.
 * Accept that occasional emergencies and unforeseen circumstances may occur, and respond to the needs of the service when they arise.
On Call
Participate in an on-call rota covering inpatient activity and emergency work. This may involve cross cover to Lung Failure and Cardiology Services. Middle Grade Doctors provide second on call to ST1-3 doctors and operate on a 1:7 rota. They are resident on-call 8am to 8pm daily with availability by telephone thereafter.
Person Specification – Qualifications / Training (Essential)
 * Full GMC registration
 * MRCP, or equivalent
 * Experience in the UK in a substantive post or equivalent
 * Advanced life support training
Desirable
 * Teaching experience with medical students and FY1/2 or ST1/2 posts.
 * Experience in the pre- and post-operative medical care of heart failure patients undergoing heart and lung transplantation or VAD insertion.
 * Experience of long‑term follow‑up of heart transplant patients including management of immunosuppression.
 * Participation in a recognised scheme of continuing medical education.
Experience – Essential
 * Experience assisting with heart failure cardiology procedures including echo, right heart catheterisation and diagnostic coronary angiography.
 * Research experience with significant publication record and contributions to major professional conferences.
 * Advanced life support training.
Experience – Desirable
 * Experience of the pre- and post-operative medical care of heart failure patients undergoing heart and lung transplantation or VAD insertion.
 * Experience of long‑term follow‑up of heart transplant patients including management of immunosuppression.
 * Participation in a recognised scheme of continuing medical education.
