Overview
As An Agency Specialist Mental Health Practitioner (Band 6), You Will Support Adults (18+) With Mental Health Needs Within Primary Care Networks (PCNs), Providing Early Assessment, Intervention, And Signposting To Appropriate Services.
Responsibilities
* Conduct strengths-based assessments and provide evidence-based recommendations.
* Work closely with primary care teams, social prescribers, and recovery navigators to support service users.
* Facilitate seamless transitions between primary care and secondary mental health services.
* Connect clients to community and voluntary sector support.
* Provide informal and formal learning opportunities to primary care colleagues to enhance mental health awareness.
* Support, supervise, and mentor junior staff where required.
Requirements & Qualifications
* Registered mental health professional (RMN, Social Worker, or Therapist) with Band 6 experience.
* Frontline experience assessing and managing adults with a range of mental health needs.
* Strong knowledge of primary and secondary care services and the wider health and social care system.
* Experience in multi-agency working, safeguarding, and risk management.
* Commitment to CPD and professional development.
* Full compliance including DBS check.
* Ability to start ASAP.
