Job Summary
We are seeking a dedicated and motivated Social Worker (Band 5/6) to join the East Hull Community Mental Health Team (CMHT) at The Grange. The post-holder will play a key role in delivering high-quality, person-centred care and support to service users with mental health needs, working collaboratively with multidisciplinary colleagues, service users, families, and carers.
Responsibilities
* Provide comprehensive assessments, care planning, and interventions for individuals with complex mental health needs.
* Manage a defined caseload of service users, ensuring that care is delivered in line with best practice and local/national guidelines.
* Undertake risk assessments and develop risk management plans.
* Promote and support safeguarding of adults and children in line with statutory duties.
* Work collaboratively with psychiatrists, psychologists, nurses, occupational therapists, and support workers within the CMHT.
* Support service users with access to appropriate community resources, benefits, housing, and social care services.
* Maintain accurate and timely documentation in line with NHS Trust and HCPC standards.
* Contribute to multi-disciplinary team meetings, case conferences, and reviews.
* Participate in supervision, training, and professional development as required.
Qualifications
* Degree in Social Work (or equivalent qualification).
* HCPC/ Social Work England registration.
* Experience working in mental health services.
* Strong knowledge of mental health legislation, safeguarding policies, and care coordination.
* Excellent communication, interpersonal, and organisational skills.
* Ability to work independently and as part of a multi-disciplinary team.
