We are expanding our City Community Mental Health Team and are seeking motivated and compassionate Band 6 Community Care Coordinators to join our Community Mental Health Team in Bradford. This is an exciting opportunity to make a real difference in the lives of service users by delivering high-quality, person-centered mental health care to the people of Bradford.
As a Care Coordinator, you will manage a defined caseload, working autonomously and collaboratively within a multidisciplinary team. You will be responsible for assessment, planning, intervention, and evaluation of care, ensuring that service users receive safe, effective, and recovery-focused support.
