We are seeking a motivated and compassionate Health and Social Care Coordinator to join the very busy Bee at Home Service. This is an exciting opportunity for someone who is passionate about improving patient outcomes and supporting individuals to live independently and safely within their own homes across Hull and East Riding.
As a Health and Social Care Coordinator, you will play a pivotal role in ensuring seamless integration between health and social care services. You will work closely with internal and external partners including social workers, community nursing teams, therapists, voluntary sector organisations and other professionals to coordinate care provision and ensure high quality person-centred support and care for individuals with a range of health and social care needs across the Hull and East Riding geographical area.
Main duties of the job
The post holder will use their extensive skills, knowledge and experience to oversee safe patient care delivery, they will also be responsible for providing day to day leadership and managerial support to the Bee at Home team working closely with the Service Manager to ensure an integrated and organised approach to safe quality service delivery across the portfolio.The service is operational 365 days of the year. This role will cover 7 days a week working between the hours of 8am and 6pm inclusive of Bank holidays on a rota basis.There may also be occasions where you will be required to work up to the operational hours of the service which is until 10pm.

Person Specification
Qualifications
* NVQ level 3 in Health and Social Care or equivalent level of knowledge and experience
* NVQ/Diploma level 4 in Health and Social Care related subject, willingness to undertake this within 2 years of appointment or equivalent level of knowledge and experience
Experience
* Awareness of team management skills
* Experience of leading a team
* Experience of working within health &/or social care setting and undertaking client assessment/risk assessments
* Experience of chairing meetings
Knowledge
* Knowledge and experience of service delivery for health or social care services
* Knowledge of relevant legislation e.g Mental Health Act (1983) Mental Capacity Act (2005), Safeguarding Adults and Deprivation of Liberty (DOLS)
* Significant knowledge of patient/client electronic care records systems (SystmOne/Access)
* Knowledge & commitment to safeguarding & promoting the welfare of vulnerable adults
* Ability to undertake or provide domiciliary care including the administration of medications during times of service pressures.
