The Frailty Nurse in a hospital setting is a key member of the multidisciplinary frailty team, responsible for delivering Comprehensive Geriatric Assessments (CGA), coordinating care, and supporting safe, timely discharges for older adults with frailty. This role spans the Emergency Department (ED), Acute Frailty Units, and inpatient wards.
Our successful candidate will have:
1. Strong clinical assessment and decision-making skills
2. Excellent communication and empathy, especially with older adults and families
3. Knowledge of frailty frameworks, NICE guidelines, and safeguarding protocols
4. IT literacy for electronic health records and virtual consultations
Main duties of the job
5. Perform frailty screening and Comprehensive Geriatric Assessment (CGA)for patients admitted via ED or acute wards.
6. Lead or participate in frailty huddles and ward rounds with medical, nursing teams and allied health professionals
7. Develop and implementindividualised care, including treatment escalation and discharge planning. Focus on hospital discharge planning and reduce avoidable admissions
8. Facilitateearly supported discharge and liaise with community services and virtual wards
9. Provideclinical leadership in frailty care and act as a resource for ward staff
10. Support education and training on frailty, dementia, delirium, and falls prevention. Promote health education and self-care strategies for patients and carers
11. Promote integrated working across hospital and community interfaces
12. Collaborate with multi-disciplinary teams (MDTs) including Medical team, Nursing team, GPs, pharmacists, therapists, and social workers.
13. Identify and manage mental health needs in older adults, including dementia and depression.
