Job Summary

Our Hepatitis C service is seeking a compassionate and knowledgeable Band 6 CNS to join our efforts in diagnosis, treatment and prevention of Hepatitis C, Including patient outreach across diverse community settings.The post holder will work closely with the CNS team, Humberside and North Yorkshire Hep C Operation Delivery Network, infectious diseases and other organisations.You will use your specialist knowledge to provide support, guidance, care and treatment to patients with Hep C.Full training will be provided by the DPOW team.Working flexibly is necessary to ensure you provide a patient-focused service at all times, ensuring that patients will be seen at times and places for their needs.You need to be passionate about making a difference and assisting with the acceptance of a new diagnosis and treatment. Working history of substance misuse is desirable but not essential.Clinical assessment and communication skills are essential. You will often be the first point of contact for patients and other medical teams and will play a key role of the multi-disciplinary team. You will need to be able to work and think creatively and proactively to reduce barriers, improve patient engagement and outcomes. Fostering an enthusiastic and trusting environment for patients.You will be based in SGH. Supported by the B7 CNS based at DPOW, working closely with the existingCNS's, and the ODN. You must be able to work independently and autonomously.

Main duties of the job


* Provide expert clinical care for patients with Hep C, including diagnosis, treatment planning, and ongoing management
* Stay current with the related latest research and treatment protocols
* Lead flexible clinics in a variety of locations to facilitate assessments, treatment, and ongoing care. Ensuring easier access for patients to work cohesively and effectively with the nurse
* Present patient cases at MDT meetings and contribute to sensible knowledgeable decision-making
* Collaborate with MDTs including drug services, supportive services and health care providers for comprehensive patient care
* Deliver education and support to patients and families about Hep C
* Engage in outreach activities in organisations, healthcare providers, and community to raise awareness and facilitate access to care the service
* A Hep C nurse must embody a compassionate and non-judgmental approach, providing care that respects each patient's unique circumstances. Demonstrate empathy, fostering a supportive environment that encourages open communication and trust
* Independence is crucial, as the nurse often manages complex cases and makes informed decisions. Coordinating with multidisciplinary teams. Strong critical thinking and problem-solving skills to adapt to varying patient needs

Excellent communication, cultural sensitivity, reliability, patience, and resilience help the nurse to educate and advocate for patients ensuring patient unerstanding and help navigate the often sensitive nature of Hep C
