Overview
We are committed to providing high quality care for the people of Bradford, Airedale and Craven. We recruit for a City Community Mental Health team to support recovery within a multi-disciplinary community mental health service for people with mental and/or community health needs.
The successful applicant will join a multi-agency team, work under direct and indirect supervision, and assist qualified Health and Social Care staff in assessment, provision and evaluation of care and treatment for patients in a community setting.
The successful applicant must be a car driver and will have access to relevant training to help them work confidently and to complete all mandatory training required for the post.
Responsibilities
* Undertake a range of delegated duties including assisting in offering practical advice, support and guidance to clients, carers and their families as appropriate to their situation, as part of an agreed care plan under the direction of one or more team members.
* Assist with assessment, provision and evaluation of care and treatment for difficult to engage patients suffering from severe mental health problems in a community setting.
