The successful candidate will be required to speak English to an appropriate standard and will need an Enhanced DBS Check. If you are living overseas or have lived overseas in the last five years you will need to apply for an overseas criminal records check from the country/countries you have resided in. You must be prepared to start work with all necessary legal documents, such as a valid visa. The Council does not hold a Home Office licence and cannot provide sponsorship for visa or immigration support. Pensions Regulations may affect pension entitlements if you are receiving a pension at the time of employment.
About the Post
We have an exciting opportunity for a qualified Social Worker to join our Hospital Social Work Team in partnership with Barnsley Hospital Foundation Trust and other partners. The role supports individuals with complex health and social care needs, including substance misuse, housing/homelessness, mental capacity issues, self‑neglect, environmental concerns and advocacy.
Responsibilities
* Work with individuals, families, carers and communities to enable informed decisions and promote independence using a strengths‑based approach.
* Complete Care Act assessments, Discharge to Assess bed triages in hospital, capacity and best interest assessments.
* Construct value‑for‑money care packages, commission and arrange appropriate services.
* Balance the needs of individuals with their assessed social care needs and health care pressures in line with our integrated care board partnership.
* Maintain high quality assessments, collaborate with agencies, and interpret case law and legislation, including the Care Act, Mental Capacity Act 2005, Human Rights Act 1998 and local and national policy issues.
Qualifications
* Recognised social work qualification.
* Registered with Social Work England or awaiting registration.
* Relevant experience working with adults, ideally in a hospital/health care setting.
* Strong communicator, flexible, creative practitioner with commitment to anti‑oppressive practice and equal opportunity.
Experience & Skills
* Experience with complex health and social care needs, including substance misuse, homelessness, mental capacity and self‑neglect.
* Knowledge of the Care Act and Mental Capacity Act, local and national policy issues.
* Skill in collaborative working with multiple agencies and interpretation of legislation.
