Joining us in Cranbrook, B.C., you’ll have a pivotal role providing social work services to patients 3 days per week at F.W. Green Long Term Care Centre. Work days are Tuesday, Wednesday, and Thursday, from 08:00 to 16:00.

About the Position:
You will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families. Working collaboratively with other healthcare providers and the multidisciplinary team, you will offer direct psychosocial care, counseling, system navigation, advocacy, education, and linkage to relevant specialized complex care resources, while facilitating client self-management.

The Dr. F.W. Green Home is a 60-bed long-term care facility. Our patient-centered philosophy supports residents’ choices, helps people function at their highest level, and provides the best quality of life possible. We house all levels of complex care, including 24-hour nursing and personal care, medication supervision and administration, meals, and laundry services. All rooms have private washrooms, as well as kitchen and dining areas. We offer various in-house programs to engage residents, including outings, community activities, exercises, music, and more, with transportation provided by the site bus. On-site amenities include lounges with TVs, public washrooms, recreation areas, laundry facilities, tub/shower rooms, secure areas, an elevator, and a hairdressing room.
