Seeking enthusiastic, highly motivated, experienced Social Workers to support young people transitioning to adulthood.

£34,434 - £44,075 per annum

Responsibilities
* Provide assessment and commissioning support for young people transitioning to adulthood.
* Work in a strength-based approach to support those experiencing learning disabilities, physical disabilities, mental health issues and ASD.
* Apply legislation such as the Care Act, the Mental Capacity Act and the Mental Health Act in practice.
* Collaborate with Children’s Social Care, education, and health services to facilitate a smooth move into adult services.
* Deliver safeguarding adults work and participate in multi‑agency procedures.
Qualifications and Experience
* Social Work qualification and registration by Social Work England.
* Good communication, time management and IT skills.
* Knowledge and experience of safeguarding adults and work within multi‑agency procedures.
* Full driving licence and access to transport; disabled employees unable to drive can use taxis.
