The post holder will be part of a team of case workers who will support the Clinical Team to provide non-clinical case management of those individuals in receipt of NHS Continuing Healthcare CHC, Funded Nursing Care FNC and other sources of health funding from Gloucestershire ICB including S117 and & Acquired Brain Injury ABI. This will include supporting providers, maintaining records, and assisting the clinical team with ongoing case management. The postholder will have responsibility for a defined case load of providers and clients providing non-clinical case management. They will provide advice and guidance to support the ongoing care provision for those eligible for health funding.
The post holder will undertake routine management and organisation of their own time and activities to support the completion of their designated role within the team.
