Responsibilities
* Provide quality, effective pharmacy services to improve patient care including technical and clinical services, and assist in education and training of pharmacy and nursing staff.
* Promote further development of the pharmacy technicians role within the Trust.
* Key Areas: Medication Management; Supply of Drugs; Patient Care.
* Demonstrate reliability in completing medication drug histories and skill in non-routine tasks e.g. advising patients/carers on medication issues or handling routine drug queries within HBTT and GPs.
* Develop and participate in Community Mental Health Team duties including medicines reconciliation, assessment of PODs, attending multi-disciplinary meetings, and liaising with other healthcare professionals to gather information on current medication and compliance status of newly admitted patients within the Home-based Treatment team (HBTT).
* Assist in the review and management of the shared care procedures within the HBTT.
* Contribute to development and implementation of policies and procedures within the department and those impacting across services e.g. Home Based Treatment Medicines Management guideline.
* Supervise and train new starters within HBTT in the provision of an effective and efficient pharmacy service.
