The Senior Health Care Assistant (HCA) will work within our community services, including the Hospice at Home (HAH) team, the Wellbeing programme, and Rapid Response (RR), which is a shared service with South Warwickshire NHS Foundation Trust. The role primarily involves working within HAH and the Wellbeing programme, with shifts 7 days a week between 8am to 8pm, and occasional internal rotation to night shifts (8pm to 8am) within the Rapid Response team.
Responsibilities include providing high-quality palliative and end-of-life care to patients and their families in their homes, collaborating with NHS and social care colleagues, and supporting patients who prefer to die at home.
In the Wellbeing programme, the role involves supporting patients, families, and carers in hospice settings to enhance quality of life, promote reablement, and encourage well-being. When working with the Rapid Response team, the HCAs support urgent care delivery alongside qualified palliative care nurses for patients with uncontrolled symptoms requiring immediate attention.
