Job Title: Urgent Care Social Worker
Salary: £33,699 - £41,771 per annum (plus £3,000 Market Forces Supplement)
Hours: 37 Hours per Week
Team and Location
Location: Worcester Royal Hospital. The Urgent Care Team is based at both Worcester Royal Hospital and the Alexandra Hospital and operates a 7-day service to support complex discharges. There is potential to work weekends and bank holidays on a rota basis with additional pay for bank holiday and Sunday working.
Role and Responsibilities
* Work with adults aged 18 and over, including people with dementia, physical disabilities, mental health diagnoses, and mild to moderate learning disabilities and/or autism, to enable safe and timely transfer of care from an acute hospital setting.
* Engage in multi-agency partnership working with wider partners.
* Work in a supportive team environment with opportunities to develop in your career.
* Maintain a fast-paced, proactive approach with strong communication skills.
* Adopt a strength-based approach in line with the 3 Conversations Model with an emphasis on Home First.
* Focus on adult safeguarding and mental capacity, ensuring organisational statutory duties are met.
* Receive regular supervision and reflection to develop practice and skills.
* Potentially work weekends/bank holidays on a rota with additional pay for these shifts.
