Due to a retirement, we are looking for an enthusiastic and confident Frailty Nurse to join our friendly Collaborative Care Team (CCT).
The successful candidate will be passionate about supporting patients living with frailty and committed to providing outstanding care in patients' own homes, ensuring their wellbeing in the later stages of life (with dedicated GP support from our Frailty Lead).
You must be a qualified nurse with excellent communication skills, as you will be liaising with a wide range of clinical and non‑clinical teams, diverse patient groups, and their families.
The role is approximately 15 hours per week; however, this can be negotiated. Salary is also negotiable and dependent on experience and skills.
Main duties of the job
* Assist medical personnel in the care of frail patients, including treatment, preventative care, screening, and patient education.
* Visit house‑bound patients and care homes; own transport is essential.

Job responsibilities
* Job title: Frailty Nurse
* Main purpose of post: To assist in the care of vulnerable patients that may be house‑bound or in a care home. The role involves assessments, maintenance of care plans, health management and vaccinations. Visiting patients using own vehicle is required.
* Responsible to: Nurse Manager and Assistant Managers
* Major duties and responsibilities:
o Monitor long‑term conditions at home or in a care home for patients on the CCT list.
o Support the partnership in strategic planning, audit and research relating to primary care services within the practice.
o Liaise with other professionals.
o Carry out injections as required.
o Carry out home visits.
o Any other delegated duty considered appropriate for the post.
o Complete and maintain care plans for patients on the CCT list, including new care home residents and new patients added to the CCT list; update annually and review ReSPECT forms.
o Assist in flu/COVID vaccination programme both at Prospect House and at patients' homes, and other vaccination programmes as required.
o Attend practice care team meetings and work with practice and care coordinators in respect of frail patients.
o Provide patient care: request blood tests and take bloods; advise patients on general health care and minor ailments with referral to general practitioners as necessary; carry out physical examinations and observations when required; liaise with the practice safeguarding team when required; refer patients to appropriate clinical or social departments/agencies; discuss social support.
o Participate in the training of new members of the practice: registrars and students on placement within the practice; participate in the education and training of students of all disciplines and the induction of all members of practice staff with special responsibility for nursing staff.
o Place of work: based at Eskdaill, with regular visits to patients in their own homes or care homes; use of personal transport is required with mileage allowance; the vehicle must have business-use insurance.
o Maintain continued education including attendance at courses and study days as deemed useful or necessary for professional development; maintain a written professional development plan and up‑to‑date portfolio meeting NMC requirements; attend regular clinical supervision sessions.
