Hours and Working Pattern: 37.5 hours per week, permanent basis. Early Shift 8:00‑16:00, Late Shift 12:00‑20:00. Rostered weekend 1 in 4.
Responsibilities
* Lead and manage a team of nurses to transform community care across the One Gloucestershire Integrated Care System.
* Manage a defined caseload of patients and delegate responsibilities to the Band 5 nursing team to support continuity of care.
* Work within a dedicated referral centre, applying critical thinking and negotiating skills to aid the triage process.
* Collaborate with other professionals and services within Gloucestershire Health and Care NHS Foundation Trust, including therapy colleagues, GP surgeries, Rapid Response and Complex Care at Home.
* Demonstrate excellent communication skills to facilitate accurate, holistic assessment of patient needs as part of a case‑management approach.
* Support and participate in training and development to enhance patient engagement.
Qualifications and Requirements
* Achieved or close to completing a Specialist Practitioner Qualification (SPQ) in District Nursing.
* Full driving licence and a vehicle for work purposes.
* Experience with electronic patient record IT systems (e.g., System One) is advantageous; training will be provided.
* Strong communication, critical thinking, and negotiation skills.
* Experience in managing patient caseloads and leading nursing teams.
