Position Overview
We are seeking a warm, skilled, and mission-aligned Adult-Geriatric Nurse to join our clinical team as a dedicated 1:1 partner to a physician focused on internal medicine and geriatrics. You will serve a small panel of patients and provide proactive, longitudinal care both in the clinic and through occasional home visits (limited to a 10–15 mile radius).

Key Responsibilities
Clinical Care & Home Visits
* Serve as the primary Nurse partner to an internist/geriatrician for 100–150 patients ages 18-100+, supporting both routine and complex medical needs.
* Conduct in-office and in-home visits for homebound or mobility-limited patients within a 10–15 mile radius of the clinic.
* Perform detailed assessments, order and interpret labs/imaging, and develop integrative treatment plans in collaboration with physician lead.
* Incorporate principles of nutrition, movement, sleep, stress, and functional medicine into care where appropriate.
* Must have exceptional clinical assessment skills
Relationship-Based, Holistic Care
* Build long-term relationships with patients and families to support sustained behavior change and well-being.
* Educate and coach patients on chronic condition management, lifestyle optimization, and health literacy.
* Practice motivational interviewing and shared decision-making grounded in patient goals and values.
Clinical Support & Coordination
* Curate and synthesize patient histories and data to support physician planning.
* Assist with documentation using AI-powered scribe tools and templates.
* Coordinate labs, imaging, medications, specialist referrals, and follow-up care.
* Handle administrative and compliance-related tasks such as prior authorizations and patient correspondence.
* Contribute to team meetings, workflow improvements, and practice protocol development.
* Participate in quality improvement and patient safety initiatives.
* Help shape a warm, inclusive, and excellence-driven culture.
Qualifications
* Bachelor’s degree in nursing from an accredited institution.
* Active RN license in Massachusetts
* 3+ years of experience in an inpatient care setting and 3+ years primary care, or geriatrics preferred.
* Passion for integrative and preventive care models. Experience with functional or lifestyle medicine a plus.
* Excellent assessment skills and experience in treatment planning and coordination of care for geriatric and adult patients.
* Comfortable performing in-home visits and traveling within a 10–15 mile radius.
* Excellent interpersonal and communication skills with a compassionate, team-oriented mindset.
