The post holder will be an integral part of the General Surgery Cancer Team(s), to assist, support and co- ordinate the diagnostic pathway for patients.
A large part of the role is providing administrative and coordination support to patients, families, consultants, cancer nurse specialists and other staff involved in the cancer pathway. The successful applicant will ensure the efficient and close co-ordination of the patient pathway ensuring robust communication between medical staff, GPs, patients, relatives, members of the public and other disciplines within the Trust and close liaison with the community staff and oncology teams.
In addition, the post holder will be able to signpost patients and carers to the relevant sources of support and information appropriate to their needs.
Main duties of the job
* Co-ordinateandtrackpatientpathways,inparticularduringthediagnosticperiod,underthe supervision of the Nurse Specialist
* Payparticularattentiontocomplexpatient pathwayswhichmayneedclose monitoringand re-adjustment to maintain optimal pathway standards.
* LiaisecloselywiththeMDTco-ordinatorensuringrobustsystemsofnotificationandwhere appropriate co-ordinate transfer of care.
* Undersupervision of theCNSdeliverpatient-centred supportandeducationasnecessaryto non-complex patients, and in some cases, complex patients.
* Supportpatientsand theirfamiliestounderstandwhatsigns, symptomsorsituationstobe aware of that would indicate concern.
* Advisepatientsandtheirfamiliesonhowto makecontactwiththe Trustwhentheyfeelthat there are delays in their pathway or require additional information.
* Advisepatientsandtheirfamiliesonhowto makecontactwhentheyfeelthat theircondition or needs have changed, including what to do out of hours.
* Ensure that the patient, where applicable, follows the optimal care pathway, and escalate if any issues arise or there are any delays

Detailed job description and main responsibilities
* Registrationof patientsontorelevant databasesensuring allpatientdata,bothclinicaland personal, is electronically and accurately recorded.
* Supportinformationprescriptiondelivery;thismayincludeprintingoutinformation prescriptions and assisting people accessing electronic information prescriptions.
* Documentandmonitorallaspectsofcarecoordinationandservicedelivery,supportingdata collection to assist with quality improvement activities such as audits of services and outcome evaluation.
* Ensureinvestigationreportsareobtained,showntomedicalstaff,actiontakenandfiledin patient case notes as soon as possible.
* Manageworkloadandliaisewithcolleaguesinperiodsofannualleaveand sickness.
* Maintainanaccuratedatabaseofpatientsincludingsystemstotrackintervalinvestigations and monitor incoming health status information from patients using the system.
* Usedatabasesand computerpackagestoinitiateandmaintainrecordsandgenerate statistics for analytical purposes.

Qualifications
* Evidence of good numeracy & literacy, equivalent to functional skills level 2 or Grade C/4 in English & maths
* Admin/Clerical background
* Knowledge of the hospital systems - careflow, iPortal etc
* Experience of working in a similar role.
Knowledge
* Proficient in the use of microsoft office, word, excel & outlook
* Previous duties in a health care setting
