Hospital Discharge
Location: West Yorkshire
Pay rate: £35 per hour
Our client is looking for Qualified Adult Social Workers to join the Hospital Discharge team. We are seeking experienced and passionate Social Workers to join a dedicated team. All candidates must have previous experience in Adult Social Care, with a good understanding of relevant legislation such as the Care Act 2014, Mental Capacity Act 2005, Human Rights Act, and Mental Health Act.
This team is responsible for screening incoming safeguarding referrals and managing complex safeguarding cases. Successful candidates will work in a specific area within the Authority, carrying out Care Act assessments, safeguarding work, multi-agency collaboration, and duty functions. This is a fully office-based role.
Working in accordance with social care legislation and local government requirements, you will support the delivery of person-centered, safe, and effective social care services. Using your professional knowledge and experience, you will make assessments and decisions about the most appropriate interventions. Your excellent communication skills will help you establish and maintain relationships with service users and other agencies to ensure services meet their needs.
You will be committed to ensuring positive and sustainable outcomes through your interventions, striving to provide the best results for service users. Your passion for supporting individuals and their families will be central to your work, with a focus on protecting the most vulnerable and working efficiently.
Ideal Candidate:
* Qualified Social Worker registered with Social Work England
* SW, CSS, DipSW, or Social Work Degree
* Experience in a Social Services Directorate or statutory/voluntary agency
* Dedicated to continuous improvement and achieving the best outcomes
* Excellent mediation and negotiation skills
* Experience working with diverse client groups
* Skilled in building meaningful relationships with children and families
* Committed to high-quality service provision
* Experience in multi-disciplinary, interagency work
* Knowledge of relevant social work legislation
* At least 4 years of qualified experience
Responsibilities include:
* Assessing risks, protection needs, and social care requirements
* Managing an agreed caseload
* Using specialist communication skills for optimal outcomes
* Developing individual care plans promoting dignity and empowerment
* Supporting and advising unqualified staff and students
