Cardiac Rehabilitation Specialist Nurse
Job Summary
An exciting opportunity has arisen within Cwm Taf Morgannwg for a Band 6 Specialist Nurse to join our Cardiac Rehabilitation team at Prince Charles Hospital. We are seeking a passionate, forward‑thinking individual with a background in Cardiology. You will work as part of a team of specialist nurses, providing a high standard of nursing care to all patients accessing the Cardiac Rehabilitation service. You will be a key member of the multidisciplinary cardiac rehabilitation team, delivering education and exercise programmes tailored to each patient in a variety of settings – hospital, clinic, community, home visits, telephone or face‑to‑face support – to empower patients to self‑manage their condition.
Key Responsibilities
* Work as part of a multi‑disciplinary team, providing a comprehensive cardiac rehabilitation programme for patients with cardiac conditions.
* Receive referrals from a variety of established routes, e.g. wards, CCU, Cardiac Catheter Lab, tertiary centres, other specialist nurses and GP's, ensuring that patients are suitable to enter the service.
* Demonstrate excellent communication skills when dealing with patients and relatives, delivering information in a sensitive and empathetic manner.
* Participate in out‑patient clinics, assessing patients’ clinical condition and identifying individual risk factors. Provide education and guidance in accordance with each patient’s needs and visit patients in their own homes when required, ensuring compliance with Lone Worker policy.
* Create specialist programmes of care for each individual patient, liaising with appropriate others as required (GP's, Cardiologists), analysing investigations and diagnostics and acting in the best interest of the patient.
* Participate in patient education groups, providing specialist education, health promotion and advice to patients and relatives; support during exercise programmes, working closely with the specialist physiotherapist.
* Work closely with the Team Leader and Consultant Cardiologists discussing individual cases and taking advice when necessary.
Essential Qualifications & Experience
* First‑level Registered Nurse, educated to Degree level with evidence of current relevant professional development.
* Experience working in or possessing sound knowledge within the speciality of Cardiology.
* Diploma or relevant experience (Cardiology/Critical Care Degree Module, Counselling, Health Promotion, teaching and assessing).
* Able to work independently or within a multidisciplinary team.
Desirable Qualifications
* Clinical History taking and Assessment Module.
* Evidence of relevant post‑Cardiac course.
Knowledge
* Knowledge of national and local guidelines in relation to Cardiac Rehabilitation and CHD delivery plan in Wales.
* Welsh language speaking/listening skills at Level 3 or above (desirable).
