Who You Are You are a qualified and HCPC-registered social worker with a diploma in social work (DipSW or CQSW) or an equivalent qualification. You have experience and knowledge in mental health work and the statutory framework. Your experience spans multidisciplinary teamwork, and you possess excellent written and verbal communication skills alongside experience in care management. What the Job Involves This role requires you to provide assessment, care management, and social work services to adults with mental health problems and their carers. You will collaborate with a multidisciplinary team to establish eligibility for services, manage care plans, coordinate services under the Care Programme Approach, and undertake safeguarding enquiries. Your responsibilities include maintaining accurate records, complying with council policies, and participating in the Approved Mental Health Professional (AMHP) rota. Your work will align with current mental health legislation, policies, and guidelines.
