Overview
Are you an established Occupational Therapist, Mental Health Nurse or Practitioner looking to work in the community? The Intensive Community Support Dementia (ICSD) team covers North and West Dorset. Driving is essential for the role; pool cars are available.
Working Hours & Location
This is a full‑time position of 37.5 hours per week; part‑time applicants of 30 hours are welcome. ICSD operates 7 days a week, 07:30‑20:00, with a rota and enhanced weekend pay. Hybrid working, including remote and base at Forston Clinic, Dorchester, is promoted.
Key Responsibilities
* Support patients in a specialist service when behaviour becomes more challenging, delivering high‑quality interventions in their own environment (care/residential or private home).
* Collaborate with CMHTs, social care and other agencies to support patients and signpost carers for ongoing assistance.
* Undertake assessments, nurse reviews, manage a small group of patients, triage referrals as they arise and coordinate shifts with other Clinical Team Leaders.
* Contribute to, and be responsible for, implementing service changes guided by the National Dementia Strategy and the Recovery Agenda.
* Operate within a virtual ward, processing information accurately and quickly.
Qualifications & Requirements
* Registered Mental Health Nurse or Occupational Therapist.
* Understanding of MHA, MCA and safeguarding legislation.
* Completion of the Prevention and Management of Violence and Aggression course before commencing work.
* Enhanced Disclosure and Barring Service (DBS) check required (the Trust covers the cost; existing DBS certificates from the update service are acceptable).
* Driving licence and ability to use pool cars.
