Advanced Nurse Practitioner/Advanced Clinical Practitioner, Community

This post is fixed term / secondment until 29/02/2028 due to the needs of the service.

We have an exciting opportunity for an experienced and innovative Advanced nurse practitioner to join the team Dwyfor Community Resource Team funded by the Dwyfor Cluster.

The cluster has proposed the development role of a Community Resource Team Advanced Nurse/Clinical Practitioner to support the cohort of housebound patients in Dwyfor/Gogledd Meirionnydd, by providing a rapid response to those in the greatest need alongside planned care in managing chronic and acutely unwell patients with highly complex and multiple co-morbidities and to recognise patients with high Frailty and initiate CGA and Anticipatory/ Advanced Care planning.

They will foster a culture of excellence and continuous improvement ensuring the highest standard of nursing care is provided, monitored and evaluated.

Main duties of the job

Key aspects of the role will include:

Working as an autonomous, advanced practitioner, with an extended scope of practice to work with the team to deliver care; providing a high standard of specialist assessment and treatment of patients. Using evidence-based principles/ current best practice and advanced specialist skills and clinical reasoning to assess, plan, implement, and evaluate interventions.

Ensuring all clinical practice is based on current research /evidence, developing and undertaking audit and research as appropriate. Participate in quality improvement initiatives, Clincal audits and research projects to enhance patient outcomes.

Demonstrating through day-to-day action a contemporary approach to Primary & community Care which is patient-centred, prudent and team-enabling.

They will maintain a visible professional presence where patients, carers and staff can turn for assistance, advice and support.

They will lead and participate in projects and areas of work, which will reflect clinical, professional and cooperate objectives.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications

  • BSc/ Grad Dip in relevant discipline
  • Master level education or equivalent; e.g. Advanced clinical practice
  • Evidence of CPD and post registration activity through portfolio
  • Extended scope skills i.e. independent prescriber
  • Membership of clinical interest group

Experience

  • Extensive experience in clinical field
  • Experience of service development or project management
  • Experience of developing protocols & clinical care pathways
  • Experience of working in an MDT setting
  • Evidence of delivering clinical education
  • Evidence of working in the community setting
  • Formal Research education

Skills

  • Advanced clinical reasoning skills
  • Ability to demonstrate advanced analytical and problem solving skills
  • Able to prioritise own workload and that of others
  • Ability to critically appraise current research /evidence and implement relevant findings
  • Excellent oral and written communication skills
  • Advanced level of negotiation and influencing skills
  • Excellent time management and organisational skills.
  • Health and safety risk awareness
  • Knowledge of clinical governance
  • Welsh speaker

Knowledge

  • Evidence of recent Masters level CPD study in relevant clinical area
  • Evidence of clinical post graduate training /education
  • Understanding of professional ethic/code of practice and their application in clinical practice

£58,379 to £65,723 a year, pro rata
