Health Visitor vacancies in various locations across Bedfordshire and Luton.
Job responsibilities
The post‑holder will work within a clinical governance framework embedded in day‑to‑day practice. CCS, Bedfordshire Community Health Services accounts for continuously improving the quality of services and safeguarding standards of care.
Take every opportunity to promote and participate in health promotion initiatives.
Act as a signpost for families to other relevant services, thereby empowering them as users.
Maintain a safe environment for patients, carers and staff where possible, ensuring evidence‑based practice is implemented.
Actively develop a specialised service or activity based on clear evidence of need, in partnership with commissioners.
Provide the core health visiting programme to the family and community.
Work in partnership with clients and families to agreed contracts to promote physical, emotional, psychological and social well-being.
Undertake a holistic assessment of clients’ health needs and produce, implement and review care plans.
Review and evaluate outcomes for children and families.
Refer clients to other agencies as appropriate, e.g., onward referral of identified vulnerable children and adults.
Develop an overall specialist protection plan with other professionals where indicated.
Undertake clinical tasks such as immunisation of adults and children as required.
Plan, organise and attend child health clinics and health promotion programmes.
Carry clinical and other equipment short distances using light to moderate effort.
Drive to venues such as clients’ homes, GP practices and children’s centres.
Have an understanding of national and local health initiatives and contribute towards achieving specified targets.
Plan and organise workload for self, support staff and respond to fluctuations.
Manage care packages meeting identified needs, possibly complex involving other agencies.
Assist in delivery of local implementation plans for the designated caseload. Work in collaboration with senior managers and stakeholders.
Undertake the role of Lead Professional where appropriate.
Qualifications and Training
* RGN/RMN/RNC/RNLD
* Mentor/998 or equivalent teaching qualification
Experience
* Experience working within a multidisciplinary team setting.
* Knowledge of the Healthy Child Programme and current best practice in health.
* Evidence of assessment of care needs, planning, implementation and evaluation of care plans.
* Evidence of involvement in project development work.
Skills and Knowledge
* Good understanding of the Safeguarding agenda.
* Ability to work with a wide range of clients in diverse settings.
* Evidence of participation in audit and/or research.
