We are looking to recruit Clinical Nurse Specialists to work in our Community Palliative Care Team. You will work in the proactive case management of patients across a locality with other Clinical Nurse Specialists, under the direction of the Team Manager. While maintaining close links with our Hospice at Home service, the Assessment HUB and the Rapid Assessment and Discharge team.


Main duties of the job

The team provides a responsive service, providing specialist palliative care advice, including the ability to deliver safe, evidenced based, symptom control advice. Alongside this, the provision of social, emotional and spiritual care for the patient and family. You will work closely with, and act as, a resource to primary care teams and other care providers with a common aim of achieving the best quality of life possible for our patients.

Am I the right person for this Clinical Nurse Specialist role?

NMC Level 1 Registration in Adult Nursing and have substantial post registration experience gained within palliative care or a relevant healthcare setting

Experience of community nursing and palliative care/oncology, but not necessarily in a specialist role

Understanding of the issues affecting patients and families at the end of life

Understanding of community healthcare service provision

Good holistic assessment skills and symptom management knowledge

Organised and have confident decision-making skills

Ability to manage own time effectively and prioritise workload/caseload for self and others

Able to inspire and motivate others to deliver excellent palliative care

Ability to deliver education, training and supervision to staff

Ability to communicate at different levels with patients, relatives and healthcare staff

Resilient practitioner with the ability to work under pressure

Job responsibilities

* As aClinical Nurse Specialist, you will work within a multi-disciplinaryteam providing specialist advice and care to hospice patients and theirfamilies
* Through holistic assessment, provideinformation, advice, psychosocial and spiritual care topatients with palliative and end of life careneeds, supporting their families and carers, liaising and referringon as necessary to appropriate health and social care professionals
* Work inpartnership with those receiving care, helping them to access relevant healthand social care, information and support when they need it. Support peopleto access, identify, appraise and interpret complex information, to enable themto determine their own future
* Useadvanced communication skills in delivering highly emotive information clearly,sensitively and unambiguously with staff, patients, families and professionalsby face-to-face, telephone and/or electronic means
* Workcollaboratively as part of the multi-disciplinary team(MDT) sharing the clinical caseload of patients/carers within thelocality for patients who have complex palliative care needs toensure efficient and timely care is delivered
* To work as part of the MDT but also work autonomously making decisionsand recommendations involving highly complex factors, seeking advice fromcolleagues or/and clinical leadership team when required, in order tomake sound decisions around palliative and end of life care
* To supportmanagement in training and mentoring a multidisciplinary workforcefrom novice to expertto work as one team
* To supportand implement any streamlined locality processes and services byembedding quality improvement methodologyincluding clear outcome measures toproduce efficient and highly performing community services
* To supportthe Hospice in building strong relationshipswith external partnersand stakeholders and promote the good reputation of theHospice e.g. local trusts, CCG, health and social care partners,businesses and voluntary organisations
* To rolemodel the values of Farleigh Hospice and support others within theorganisation to do so by recognising positivebehaviours, challenging poor behaviour, encouraging andsupporting change
* Tobe visible and credible with the ability to listen and respondto staffandtopatient and family issues and insights
* Tocommunicate effectively with all staff across the organisation to promote oneteam, one vision and collaborate with colleagues as appropriate to deliver this
* To Workas part of a team and provide clinical cover during the absence of other teammembers and ensure the priorities of the service are met especiallyin times of high demand. To adapt to the pressures of changing demandsday to day
* To deputise for the Locality Care Team Manager when necessary in relation toclinical leadership
* Toparticipate in the development of Hospice response toany identified changes in the supportive and palliative care needs of ourpopulation and engage with patients, carers and external professionalspositively sharing any agreed change in service offer
* Tomdemonstrate skill in conflict resolution and use competent negotiation skillswhen dealing with difficult or challenging situations such as managingpatient/carers unmet expectations
* Toengage and collaborate with external partners todevelop locality-based joinedup services using quality improvement methodology
* Tomanageyour own performance as well as identifying existing skills andcompetencies and ensuring that professional development plans are tailored tothe changing capability requirements of the organisation
* Toprovide specialist advice, education and support to professionals/staffregarding the management and care of people with specialist palliative careneeds
* Topromote the continuing development of specialist knowledge, skills andattitudes to more junior members of staff in relation to end of lifecareincluding palliative care and frailty competencies
* Tosupport, guide and motivate junior members of staff todeliver their best through effective use of the staff support matrix,through cluster group leadership as required
* Tomaintain own professional standards and to lead by example by participatingin supervision, professional registration,and the delivery of personaldevelopment plans ensuring compliance withprofessional developmentandmandatory training at all times
* Toparticipate in EOL and Palliative Care educational programmes. To givefeedback from attended education programmes/study days to themulti-professional team
* Tocarry out coordination duties as requested
* To assistin recruitment to ensure appropriate staffing levels are in place througheffective recruitment and rostering


Person Specification


Skills/Ability/Knowledge

* Proven ability of successfully working with people and handling difficult and complex situations
* Ability to work under pressure, manage own stress and emotional responses and be aware of stress in others
* Ability to use a range of IT systems and software, including Word, Excel, PowerPoint, Office 365 and electronic patient record systems (such as SystmOne)as well as virtual systems e.g. Zoom, MS Teams
* Proven ability to reflect and critically appraise own performance
* Ability to motivate self and others and to lead by example
* Able to effectively resolve professional and ethical issues
* Ability to prioritise issues and tasks and manage own workload
* Excellent oral/written communication and interpersonal skills
* Effective analytical and judgemental skills to deal with complex cases


Qualifications

* Nurse registration
* Evidence of continued professional development
* Advanced Communications Skills Training
* Leadership training/qualification
* Non-medical prescribing and Advanced Clinical Assessment skills


Experience

* Demonstrable experience in the delivery of complex specialist palliative care
* Experience of holistic assessment and complex symptom management
* Experience of multi-disciplinary team working
* Experience of working unsupervised, manage different patients and make decisions related to complex care planning
