An exciting opportunity has come up for Advanced Practitioner position on a temporary 6-month basis in the Walworth Community mental health team. This will be to cover the current staff in post, who is taking off 6 months to pursue education training.
This is an opportunity for anyone interested to enhance their leadership skills and have a pivotal role leading a busy and dynamic team. The Advanced Practitioner will support the clinical team leader in providing clinical leadership and managerial oversight for the allocated CMHT. They will support transfers across the community mental health teams (CMHTs). This includes taking a lead in clinical decision making, supporting the referral pathway in an out of the CMHT, as well as deputizing for the team leader.
We are looking for dynamic and enthusiastic candidates, with strong leadership skills and a commitment to delivering high standards of care.
Main duties of the job
You will be expected to work with the service lead, team leader and team members.
You will also provide support to care coordinator's with complex cases using evidence based/client centered principles, as well as supervision and appraisal for staff in the team. You will oversee the referral pathway within locality CMHT, this includes: helping to ensure that the team consistently meet the Waiting Time Standards, having oversight of referrals coming into the team, and helping to ensure that clients are discharged from the service per agreed care plan. To develop and maintain good communication between the multidisciplinary team and other agencies, those referring to the team such as GPs, other community teams, ward services and home treatment teams.

Experience
9. Extensive post registration experience in adult mental health in a variety of Mental health setting
10. Experience of undertaking a leadership role
11. Experience of working with people experiencing severe and enduring mental illness
12. Experience of providing supervision in a clinical setting
13. Experience in managing safeguarding concerns and risk
14. Ability to analyse complex issues/problems, provide guidance and make decisions (A/I)
