As our Support coordinator, you’ll be at the heart of supporting our customers who have complex needs including mental health issues, substance misuse and offending behaviour, to develop their skills to live more independently in the community. You’ll make a big difference each day as you help our customers achieve their hopes and aspirations. Amazing we know!
Our customers live in our main accommodation as well as living in dispersed properties, where we have 10 flats in the community. We aim to help our customers live independently.
Typical day as a Support Coordinator
1. Creating support plans with your customers and coordinating our team of Support Workers working to the plans.
2. Supporting our customers who have complex needs including mental health issues, substance misuse and offending behaviour Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
3. Carrying out risk assessments, support planning, goal setting and regular reviews.
4. Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring
5. Passion to support our customers to live their best life, working collaboratively with an eye for detail.
6. Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
7. Experience of coordinating and assessing customer referrals.
8. The ability to work on your own initiative, remain calm under pressure and have a resilient approach
9. To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage!

You’ll work alongside our fabulous team who are super passionate about supporting customers to reach their goals and celebrating their achievements. To us we're not just a team we’re a ‘work family’.
Our managers are Stacey, who has worked in various roles within Home Group for 12 years, and Benjiee, who has been also had various roles in their 5 years. There are 20 of us in the wider team and we don’t just love to support our customers, we’re there for each other too!
Job details
10. You’ll work on a rota basis between the hours of 8am and 10pm and take your turn working days, evenings, weekends and bank holidays.
11. We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!
12. Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
13. Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
14. You’ll need an Enhanced DBS check done and we pay for that.
