Job Overview
We are seeking a dedicated Care Coordinator to join our team. The ideal candidate will be responsible for ensuring the smooth and efficient delivery of high-quality domiciliary care services. This role involves scheduling and coordinating care visits, managing client and staff relationships, and ensuring compliance with regulatory requirements. The Care Coordinator plays a key role in ensuring that clients receive person-centered care that meets their needs and preferences while maintaining the continuity of service.
Key Responsibilities
* 1. Care Planning and Coordination
* Develop, maintain, and update care schedules to ensure all client needs are met in a timely and efficient manner.
2. Staff Management and Support
* Allocate work to care staff and communicate their schedules clearly and promptly.
* Provide guidance, support, and supervision to care workers, addressing any concerns or issues they may have.
* Participate in staff recruitment, induction, and training to ensure a skilled and motivated workforce.
* Monitor care staff performance, ensuring adherence to company standards and regulatory requirements.
* Conduct spot checks and observations to maintain the quality of care delivered.
3. Client Relations
* Build and maintain positive relationships with clients and their families, ensuring satisfaction with the care provided.
* Act as the main point of contact for client inquiries, complaints, or concerns, resolving issues promptly and professionally.
* Regularly review client feedback to identify areas for improvement in service delivery.
4. Compliance and Quality Assurance
* Ensure all care services are delivered in compliance with CQC (Care Quality Commission) regulations and other relevant standards.
* Maintain up-to-date records of care activities, including risk assessments, client reviews, and incident reports.
* Participate in audits and quality assurance processes, implementing recommendations where necessary.
* Stay informed about changes in legislation and best practices in the care sector.
5. On-Call Duties
* Participate in the on-call rota to handle out-of-hours emergencies and provide support to care staff as needed.
* Respond to urgent client or staff issues, including coordinating cover for unplanned absences.
* Match care workers with clients based on skills, availability, and client preferences.
* Monitor daily schedules and make adjustments as necessary, including managing last-minute changes or emergencies.
* Conduct initial client assessments and reviews to develop person-centered care plans.
* Ensure accurate documentation of care plans and all associated records in line with company policies.

Qualifications and Experience
* Previous experience in a similar role within domiciliary care or health and social care (preferred).
* Level 3 Diploma in Health and Social Care or equivalent (or working towards it).
* Strong knowledge of CQC standards and regulations.
* Experience in scheduling and rostering systems (e.g., Care Planner, PeoplePlanner, or similar tools).
* Full UK driving license and access to a vehicle (preferred).
Job Type: Part-time
Pay: £23,660.00 per year
Expected hours: 21 – 35 per week
