Job overview
The post holder will provide a professional, proactive, and sensitive service to drug monitoring patients and their relatives. Will act as point of contact for patients, carers and the wider healthcare team to provide high quality patient care through proactive identification of needs which require clinical knowledge using approved tools and procedures.
To work with Neurology Specialist Nurses and the wider healthcare team, acting as a resource to ensure appropriate services, to maximise patients' health and quality of life, and improve healthcare delivery. Responsibilities include coordinating prescriptions, managing administrative functions within neurology pathways, arranging investigations, appointments, and multidisciplinary team discussions in a timely manner, ensuring targets are met, and managing service demands flexibly.
Main duties of the job
* Act as a contact point for patients, carers, and healthcare teams, triaging calls and responding per protocols.
* Handle situations with empathy, calmness, and support.
* Escalate issues to healthcare professionals appropriately.
* Organise diagnostic tests and treatments proactively.
* Coordinate with teams to ensure seamless patient pathways.
* Manage own caseload, including outbound calls, planning care, and monitoring service effectiveness.
* Maintain accurate records of patient contacts for communication.
* Advocate for patients and facilitate issue resolution.
* Collect patient information for quality care and pharmacy processes.
* Handle daily administrative functions, prioritising workload and reviewing practices.
* Maintain and organise the drug monitoring email account, delegating as needed.
* Assist in developing patient education and information.
* Utilise software (Excel) for documentation and monitoring of patient care and service delivery.

Qualifications
* GCSE/O’ level in core subjects or equivalent experience.
* Proficient in IT systems like Word and Excel.
* HNC or NVQ Level 3 or equivalent experience.
* Knowledge of databases and spreadsheets.

* Experience working with multidisciplinary teams.
* Ability to build effective relationships internally and externally.
