Make a tangible impact on patient care at the frontline of medicines optimisation.As a Clinical Pharmacist, you will be a key member of the multidisciplinary ward team, delivering a high‑quality, patient‑centred clinical pharmacy service to inpatients. Working proactively alongside ward clinicians, Practice Pharmacy teams, GPs, and carers, you will play a pivotal role in ensuring the safe, effective, and cost‑effective use of medicines.You will use your clinical expertise to support prescribing decisions, optimise treatment outcomes, and improve medicines safety, while contributing to seamless care across the patient pathway. This is an opportunity to work collaboratively, add real value to patient care, and influence high‑quality clinical practice on the ward.BaseThis role is based atSwindon Intermediate Care Centre, a modern healthcare facility providing high‑quality intermediate and inpatient care.
