Come and join our dedicated and friendly team Transfer Unit

We are looking for caring, compassionate, enthusiastic and self-motivated Healthcare Assistants ready to help us to provide high quality care and excellent patient experience.

The Transfer Unit is open Monday to Friday 7:30am - 19:30. The unit can accommodate up to 16 patients both seated and in bed.

The unit is staffed by Registered Nurse and Health Care Assistants. The unit support patients who have been deemed No Criteria to reside from ward areas or those have received treatment in Emergency dept but do not require admission into an acute bed. As a Healthcare Assistance you will be working very closely with the nurses and the rest of the multidisciplinary team to ensure high quality care and safe discharge.

The Healthcare Assistance role is key within our patients' discharge as you will be providing, supporting and encouraging patients with personal care, nutrition and hydration, mobility and safety whilst awaiting discharge. You will support the RN with transferring patients to the unit and collecting medications and equipment from across the trust.

Main Duties and Responsibilities

As a healthcare Assistant you will be expected to work within our dedicated multi-disciplinary teams and be involved in providing individualised care, assisting with patient hygiene and dressing, and meeting the nutritional needs whilst maintaining their dignity and respect. Contributing to the safety and cleanliness of the environment whilst keeping good documentation.

Communications

* To communicate effectively with the multi-disciplinary team, under the supervision of a registered nurse.
* You will support the RN with transferring patients to the unit and collecting medications and equipment from across the trust.
* To carry out routine administrative/telephone related tasks.
* To communicate effectively when giving/receiving verbal and written information.
* Ensure patients have all property and aids with them on discharge.
* Communicate with patients friends/ relatives around discharge plans.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Please note that whilst the trust does offer sponsorship this role isunfortunately not eligible for visa sponsorshipunder the UK Health and Care Worker visa route. This is due to the position not meeting theminimum salary threshold of £25,000 per annumand/or the requiredskill levelset by UK Visas and Immigration.

Experience

* Currently working or has worked in health or social care
* Worked in community and/or clinic settings
