Hospital Discharge Team
There is an exciting opportunity within the Hospital Discharge Team for an Independence advisor to work 37 hours per week across Monday to Friday.
About The Team
The Hospital Discharge team are responsible for completing Care Act assessments and support plans for people within the hospital setting, both acute and community, to facilitate safe discharge plans within the duties of the Care Act 2014. There is a strong focus on Home First and working with a strengths‑based approach within a strong MDT ethos and also working within Discharge to assess pathways that are unique within Southampton Hospital Discharge Team.
Alongside this the team complete other statutory duties for example working under section 42 of the Care Act completing safeguarding enquiries and carrying out work within the remits of the Mental Capacity Act assessing the capacity of the individuals we work with and supporting decision making.
The strong emphasis on Home First enables people to remain in their homes and living as independently as possible which comes along with working as an MDT to support holistic assessments. We also offer a supportive role to carers and ensure they feel enabled in their role as they are invaluable in their support to our patients.
The team are continuously involved in lots of exciting developments within the hospital discharge system and there will be an opportunity to be part of these changes and provide input into the development of these changes, including the introduction of a new recording system.
The team are currently based at University Hospital Southampton and carry out flexible working between this office base and home with opportunity to also work in the Civic Centre also. There is a requirement to work from the hospital for duty shifts at least once per week and to work in the office to complete in‑reach visits on your cases alongside a need to in‑reach to the Western Community Hospital and to complete reviews in the community post discharge. This will require visiting clients in their own homes and care settings.

Salary: The salary band for this role is £37,280 - £40,778 per annum. The starting salary is £37,280 with annual progression through the salary bands.
