Job summary
The post holder isresponsible for the delivery of general practice nursing care to the practicepopulation. Supported by a senior nurse within the practice, they will delivercare within the boundaries of their role, focusing upon supporting patients tobe healthy, monitoring of long-term conditions, health prevention and screeningactivities. They will work collaboratively with the general practice team tomeet the needs of patients, supporting the delivery of policy and procedures,and providing nurse leadership as required. In addition, the post-holder willbe required to support the multidisciplinary team, ensuring the strategicobjectives of the practice are met.
Main duties of the job
The successfulcandidate must have the ability to develop, implement and evaluate individualtreatment plans for patients with a known long-term condition. Support patientsto adopt health promotion strategies that promote patients to live healthily,and encourage principles of self-care, including cervical cytology. Implementand participate in vaccination and immunisation programmes for both adults andchildren. In addition to the primary responsibilities, the practice nurse maybe requested to: Support the multidisciplinary team as required, Partake inaudit as directed by the Clinical Audit Lead and Support the Practice Managerin reviewing and updating clinical standard operating procedures, protocols andpolicies

The clinical teamcomprises of GP Partners, Salaried GPs, Advanced Clinical Practitioners (Nurse,Paramedic, Pharmacist), Practice Nurse,Healthcare Assistants and Phlebotomist.
The practiceoperates on a team based approach with regular meetings to share best practiceand ideas. Team objectives are clearly stated and measured against national andlocal criteria.
